Access Filtering list

Anybody can help me on the access list configuration?
I need to permit only certain port to send traffic from hq to remote and vice versa.The port is a tcp and udp port 5631, 5900, 1433 and 1352.My customer is using a dial up connection.Since the bill is so high so they want to permit only these port to go through.

Any idea?...Thanks
 
access-list 101 permit tcp (source address) (source wildcard mask) (dest address) (dest wildcard mask) eq 5631
access-list 101 permit tcp (source address) (source wildcard mask) (dest address) (dest wildcard mask) eq 5900
access-list 101 permit udp (source address) (source wildcard mask) (dest address) (dest wildcard mask) eq 5631

.
.
once the list is written, simply go to the interface you want to apply it to and then do the following:

ip access-group 101 (in/out)

in or out depends on what traffic you want the list to apply to
