AA hangs up on transfer

We have a Merlin Mail VMS Rel 3. "Sometimes" when a caller transfers from the AA to an extension (or by pressing a prompted number), the system simply goes to dead air and eventually hangs up on the caller. There is no "transfering to another call" or anything else.
That same caller can call back and after 1 - 5 tries eventually can get "someone". I have been able to duplicate this (randomly) - I cannot find a pattern. It can happen to any extension.

Any help will be appreciated.

make sure all voice mail ports are assigned to calling group. check touch tone receivers on all cards by dialing *04 then tt # (01, 02, 03) then dialing all the #'s on the dial (1,2,3,4,5,6,7,8,9,*,0,#). You should hear two or three beeps if tt receiver is good. Do this to all cards having tt receivers.
 
Thanks so much for the response. I am not sure how to do the check you suggested. I tried going to the switchboard and dialing *04 then tt then 01. I tried *04 then 01. I tried *04 then pressing the extension on the left side of the switchboard. No luck on any of these. I also tried from my own phone. If you could help with more detailed instructions, I will greatly appreciate it.

Some more details about the problem...
This morning my system was completely open (no calls). My Sales Manager called in and dialed an extension - the system went to dead air. No message, no hang up - just dead air for about 30 seconds. He hung up and called back, dialed 0 and it went to the switchboard. I cannot see any pattern to when this fails. I had suspected it was when the system was overloaded, but this morning disproved that idea.

Is the calling group for your voice mail set to Linear or Circular? It might help point out if you are hitting a bas VM port.

Sorry, You need to be at a single line set to do the tt receiver test. If you don't have a t/r card you can't do it.
