9508 echo on speakerphone

I have a IPO R9.1.6 with 9508 phones looks like R59 firmware for the phones. The customer is complaining of echo while on speakerphone but it goes away when they pick up the handset. They are also complaining about the same echo on a Polycom conf phone. The 9508 phones have had the echo canceler turned off as well as the AGC. The echo the customer complains about is on the remote end not at the IPO. I am not sure where to look now as I have never had echo before on digital phone with a PRI.
i have read all the tips about the echo canceler.

try backing off on the volume of the phone itself as this may help
 
They seem to be set to the middle of the setting, not sure that it should cause an echo if they are not increasing the level from factory. Also it seems like the echo happens when you are speaking at the same time as the caller.

Echo Canceller:
Press HOME (Key with The A on the button, exact center of the phone)
Press arrow down button to ADVANCED OPTIONS, press OK
Press arrow down button to AUDIO, Press OK.
Press arrow down button to ECHO CANCELLER, Press CHANGE to turn off.
Press SAVE.
Press BACK, Then EXIT.
 
upgrade to latest 9.1 and see if the firmware upgrade on the phones solves the issue.
The firmware will go up to R60 then
