9.0 SP2 server edition SIP issues. Outbound working, Inbound not showing up in monitor at all

I have an IP Office Linux server running 9.0.200.860. Sip trunk is patched into LAN2 of the IP 500v2 expansion. I'm able to make outbound calls but unable to even see inbound calls hit the system in monitor/ssa. The sip provider debug shows a 404 then 604 error. Any ideas on what the issue could be?
