4503 - show mod doesn't show IOS on MOD 3? 2

Q1) MOD 3 not showing IOS firmware....Is this normal?
Q2) MOD 3 keeps shutting down its ports? I checked spanning tree and found the following: (which may lead to the ports being shutdown).

1.)WS-X4418 is a line card, so it does not have an IOS. IOS code resides on the supervisor cards.

2.)You listed 1 port in blocking state, but you mentioned ports. Check port-security to see if there are some violations that would cause them to go into err-disable state.

It's perfectly normal for 1 port to be blocking if you have mutliple links connecting to other switches for redundancy. If you don't have this though and it is blocking, you should check to see if someone accidently introduced a loop that you aren't aware of either through incorrect cable connections and/or recently added hubs or switches.
 
Do a show cdp neighbor and see if the switch sees itself on more than 1 port , if yes I bet 3/2 is one port which means you have a wired loop between 3/2 and qnother port on the switch and spanning tree has blocked that port .
