406V2 w Pri (Qwest) calls drop mid stream mystery

The problem spans ports across all modules, there is a PRI from Qwest, units are grounded and PRI parameters were checked with Qwest. Qwest checked the line and spotted what they called Layer 7 errors, here is some of the output from their test, which they point back to the 406V2 and Pri card as the culprit for dropped calls and these errors. Any ideas what could cause this and possible cures?

We are getting Catalyst and Avaya involved next. I'm sure that they'll have us send in some traces. Any ideas?
 
I think that I'm having the same issues at one of my locations?

Anyone have any ideas?
 
You may want to try knocking down the 'Haul Length' setting on the PRI to -7.5 db. Sometimes a PRI comes in a little too 'hot' for the IPO. Z
 
I checked and Mine is set at T1 and I don't have that as a option. Mine is set to 0-115ft.
