3.0: IP406 w/ T1 does not detect when call disconnects, no caller ID

IP406 configured with T1: I suspect both of these are related, therefore one post. The system apparently doesn't recognize when someone hangs up. For example, can call someone, then other end hangs up and I get dial tone until operator msg tells me to hang up. Same on voice-mail if someone calls and leaves mesg, get up to 2 minutes of dial after. But this is NOT limited to voicemail prob as first described above. It happens on regular voice calls when voice-mail not involved.

Also, Caller ID is not appearing on phone sets 5420. Doesn't seem to be showing up on system in the system logging either. Carrier is supposedly passing this info.

Any ideas / configuration suggestions?
 
Had service provider come back out and configure equipment to ground start instead of loop start. Once IP Office and Adtran 600r were configured for ground start, disconnect problems went away.

Opening separate thread on caller-id.

Just updating thread in case anyone else ever uses Adtran 600R by provider to connect to IP Office via T-1 configuration.
