2250 Console issue

We have a 2250 attendant console that is acting wierd. The receptionist ended up being conferenced into a call and she could not disconnect it. The conference was initiated internal and included external persons. The person did not intend to include the receptionist, we dont know if she was included by error or because of something buggy with the phone/switch. Can someone take a look at this scrape for anything wrong? Is there something else i should look at? I have no ideal what to look for so any help is appreciated.

This is not the first issue we have had with the console, we have had issues here and there.

after a call is placed to the console the calling party can not tear it down, hang up on her and see is still there. makes a conf call to her tough to tear down unless she set it up. i would spend some time at the console postition before i replace the hardware, might be a headspace problem. software is correct

Thanks for response John. I am going to do some testing to try and recreate this issue. Is a headspace problem the same as user error?

Or sometimes referred as an "I-D-10-T" or PEBCAP (Problem Exists Between Chair And Phone)
 
PEBCAP that's a new one that i will use, in vegas we had a code for everything back before centel got bought out.. 31 was a loose screw and 21 was a faulty handset, so a 31-21 was a loose screw holding the handset... dispatch never knew the diff..
