For the life of me I cannot remember or seem to find the name of a cable that on one end can be connected to a 110 punch down block and on the other end of the cable is an RJ-45 connector. Thanks in advance for the help.

If I'm reading this right you're talking about a patch cable used to cross-connect from a 110-style termination block to a 8-position, 8-conductor modular port (aka RJ-45).

If so you only need to request a 110 to RJ-45 patch cable from any distributor, they'll know what you're talking about. Make sure you tell them the category cable, how many pairs you want on the 110 side, and the pin-out you want for the RJ-45 end.

Hope this helps.

It sounds like a patch cord to me. Panduit, Siemon, Systimax, and Leviton would be the name brands to start with. I'm sure you could get off brand as well.

Is that the MODtap adapter? Do you have a part number for those? I haven't been able to find one for some reason.

Siemon company has 1,2,3,and 4 pair 110 adapters that push right onto the block and give you an RJ-45 right there. I found them on this site. just scroll down to 110 Adapters. I fyou're going with 4 pair, be sure to choose 568B if you're dealing with 2 pair phones.
