Does reinstalling the Avaya Softphone give you the same problem? I would try to remove the software than install it fresh. Maybe installing it again may pick up something that your upgrade changed?
I have the same issue with Comcast users only. The users on Charter work fine. At one time I did get one of the Comcast user's IP phone to come back online by cycling power to the cable modem. However, this user only worked for a month and it was back down again. VPN is still established but...
Check your gateway on the medpro. It may also help to add "no ip redirects" to your router. This addition to our router resolved the same issue you are having.
