Whether it is a combobox or an option group doesn't matter. Both fulfill the same requirement. I am interested in the value stored in the form. How do you change the numeric value recorded for a yes response to 1 and for a no response to 0?

Thank you for the suggestion. I looked into it in the help menu of Access. It looks like using option groups will place all the entered values, (1 through 5), into a single field. Is this the case? If so, is there a way to separate the entered values into 5 fields?
