Why don't you try adding the user in the USER ACCOUNTS and set the appropriate level access. Are you using NT Domain or 2K Active Directory?

Would be the second suggestion (No.2 DNS Resolution Issue) be applicable to NT Domain? Should DNS server be local to the network?

Try user: administrator then the password:blank or empty, So that you won't have to re-install your windows.
