:) you will see more problems like these very often in these ESIM's. Lots of bugs, and they dont seem to be interested in clearing them up.
But, what I see from your questions(not only these,many more), Looks like your fundamentals on Cisco is right.

What they mention in the books is just the guide lines.the access lists should be applied where u intend to filter the packets. Now theres again the direction of the filter too ie; inbound and outbound. So practically the theory in these books makes no sense.
But, If u're preparing for the CCNA...
Prepare for the lab......then it becomes easy for the Written....I would appreciate if u can have some hands on the Routers and Switches before u can try next time...
The book(hopefully not the New CCIE book from Cisco Press-Author: Bruno) u used to study is good to prepare...but it all depends...
