Is this only happening at your remote sites? Are the servers at these sites acting as Domain Controllers to authenticate users there? Since you mention that this happened after a policy change, you may want to make sure your primary PDC is replicating correctly to the BDC's at those locations...
Do you notice if the clients are having any trouble connecting to any of the servers. May want to double check all the ICA connection settings on the servers to make sure they match.
Also check any network security or group settings for the published apps to make sure everything looks good.
