I should clarify I know how to do the style separator trick (CTL-ALT-ENTER on the heading), but I want something that automatically formats as I am working a large document.

What is the format of the clip you are editing? AVI, MPG? If it's compressed format like MPG - that is likely the source of your issues.
You didn't specify the format of the clip you are trying to play. If it's compressed video (e.g., mpg), then all version of Premiere will have trouble with playback because it must interpolate the compression.
What version of Premiere are you using? DVD players don't play AVI files, they must be MPG-2. So what encoder are you using to convert AVIs to MPG?
