The BES does not have any functionality to disallow sites not in device favourites. Te BEs is not aware what favourites people have stored. Are you sure there is no proxy server in between the BES and the internet blocking these sites? If so, can you bypass the proxy for testing?

Yep, install the new one and then gradually move users. You can use Desktop Manager to backup and then restore their data after the wipe. You will need new SRP and CALs for the new server though. I believe you can purchase a migration kit for this.
If you install 4.1.6 new, then you have to wipe devices and reactivate each one on the new server. If you do an in-place upgrade of 3.6 to 4.1.6 then you don't. The MR is a rollup of fixes for SP6, I beleive MR5 or 6 is the latest one.

If the option is enabled for PIN & SMS message logging (it is not enabled by default, the Admin has to enabled it) then the contents of the messages are logged on the server as well as date, time, sender etc.

That would be Active Directory, not Exchange. Those types of settings don't change by themselves. Are any of your admins making changes, mayve even amending Group Policy settings?
Could be any number of things. Does she get a red X when trying to send? If so, open the message, scroll to the top and read the error.
Alternativley, search for her name in the MAGT logs on the BES, after she has attempted to send, and see what errors are logged for her account.

Yes, a UK user should have the Smart Dialing Country Code set to +44 and a German user should have it set to +49 etc etc
A non-US user would have to be dialing +1 for any North American number. So if you were looking at amending your database, all NA users should have +1 in front of their phone...
