You need to make sure that IIS virtual directory has write permissions AND the directory itself you are writing too has write permissions, if you are not using Windows Auth., then the user will be anonymous and you will need to grant Everyone (or the anon user) write permission to the directory...
My guess is that because Citrix is so finky about latency, that the VPN client might be holding back the connection UNTIL there is traffic to send. If this is the case, it could be that the ICA client cannot see the citrix server the first time, but the second time, the VPN client has initiated...
One way to check the Windows vs Citrix is try logging in with RDP (terminal services client) and see if you get the same message. If you do not, then it must be a Citrix licensing thing, if you do then it must be a Windows terminal licensing thing.

Citrix can be very finiky about latency. Depending on how and what type of VPN you are using, the encryption and decryption could be causing latency problems which would make the server seem like it was there and wasn't there sometimes. You may need to do some eval of your VPN performance.

This is possible with the server->client redirection in Citrix XP Feature Release 2. This makes internet URL's open in the local web broswer of the client. Although I have seen some instances where the application itself can cause the redirection to fail (but very rare).

You really need to combine all the suggestions above. Check the nfuse.conf file for where the NFuseIcons is RELATIVE to your webserver directory (www.yoursite.com).
If using IIS, create a Virtual Directory that imitates the NFuseIcons setting in the nfuse.conf file.

You might want to check the Management Console for the number of days you have left in eval period. Make sure that the number is greater than 1. Also, try logging in with terminal services RDP versus Citrix ICA. That might give you some errors to help you find the problem.
