Suggestion, post the URl for the site so we can see whats going on. Also, off the top of my head, most of the email links are just for that, email. You may have to combine forms into database for proper receipt.
Try constructing background stuff in DW, either make form in FW and export/import separately or use one of the extensions available. Also, check your layers properties, may be hidden, lower z value. The text stuff will be better done in DW with an extension, or manually if you know how.
Check cell padding, spacing etc in the properties dialog. Manually change values in HTML source if needed. What browser are you using? NN can be a problem here.
