You should always register locally first. It makes more financial sense to do so. For example, If you have three sites with 3,600 users (1,200 per site). On a sunny day, if you had every phone register to the main site, you would need ~12 C-LAN's at the main site to handle all phones...
Remember, these phones are natively supported on CM4.0 and beyond. You can now set the VM button per station or continue to deliver the value via 46xxsettings.txt.

The 96xx phones download files via HTTP not TFTP. You need to put the firmware files in a valid HTTP root directory. If you're deploying a lot of phones (over 200) I would think of installing a dedicated HTTP server.

Well there are probably a half dozen "known issues" with ESS's that were resolved going from CM3.0 to CM3.1. As far as IPSI over WAN, that is an engineering issue not CM release issue.
