I had similar issues with an Avaya IP Office, it was to do with the Network Topology config on the IPO.
Got it working on a few Panasonics and loads of NEC SL2100/SV9100 however

Out the box PBX's like Avaya IPO500, IPEC UCP100, etc days will be numbered
I believe Avaya only do 1 digital handset now from memory...There is little need to have an all in one box now

Something funny going on with your VPN routing,if it's a small amount of IP Phones just put them all in different P2P group.

Is the remote branches got IP phones that need to talk back to the NS500?
You either need to do some NAT rules or if mikrotik can do VPNs between sites, we need more info to help you

It don't believe it will change your config on the extensions, just take backup , power off then swap your card.
Worst case scenario it renumbers your base extensions which is a 2 second fix but I don't think it'll do that.

Your combo ATM card will be perfect, just make sure you don't already have 2 combo cards (BRI combo card for example), IPO can only take 2.
I have not done this on Avaya with a gateway but I am sure it will be fine, it's just another point of failure.
