Open up CCM/CCA Configuration Manager and browse the users; find the user in question and you should see which .mdb file it's using e.g. 0000XXX.mdb
I usually rename it to 0000XXXmdb.old and copy the default one then rename it to the same file name (0000XXX.mdb), make sure this is done...

Thanks for your response, when the headset is removed the fault still occurs so we can rule that one out. The fault is intermittent but the users are very vague as to when it's happening; they say randomy during the day.

Connected to gatekeeper ELU32 (ROF1375428/1 R6A) running firmware R7A.
