Thanks for all the suggestions. It turns out that DHCP is working again on the phone. I'm not sure what did it except that I installed the latest patch for BCM 3.7. The phone is now getting ip addresses from the network's dhcp server.

I was also thinking lack of IP addresses available might be the issue; I tried to deregister one of our IP phones (currently working) to free up the IP, before trying once again to reconfigure the new phone: same results.
