I have installed lots of the Wireless Handsets and have never had the problem of the on-hook, off-hook button not working on the Wireless Handset, so I would also guess a defective unit.

This is also a problem that we reported to Avaya more than a year ago. If the Digital Phone is connected to the ports on a Base Controller, the true DTMF Tones are played. If the phones are connected to a DS Expansion Module, it sounds like you are pressing the buttons on a toy piano...
We have several of the 5330/5340 phones and also use SpectraLink and have not seen that particular issue with noise when calling between them.
We are now running Release 8 Beta, but we did not see that problem on any previous release either.

Thanks for the input to everyone. We ended up switching the Linksys units for a Snapgear 300 from Secure Computing and everything seems to be working fine now.

I have installed both and like the quality of the Mitel system over the IP Office. Here are a few points:
