If it only needs to write to the file once then you can have a directory where users have write permissions but not read. That way they/the program can save the file but not reopen it.
WinInstall LE is free and on the Windows 2000 cd this can be used to create MSI's, but i highly recommend WinInstall LE 2003 it's still free and can be downloaded from http://www.ondemandsoftware.com/ it's a great piece of software.

You can set Terminal services to administration mode when you install it, i think it's the default anyway. You will know if it's in application mode as it will imform you at every oppurtunity.

We use TSWEB 2003 and in my experience its ultra reliable, you do need IIS but it doesn't have to be on the server you are trying to conect to. As long as its in administration mode you don't need any licences as there are 2 free admin licences.

Not really related to the thread but if SUS is running in a domain and the group policies to specify an intranet update server are set, is a reg edit still required to get it to work.
We use group policy to distribute MSI files to our machines and it works fine for out 140 stations.
You can set them to install on boot up or by file asociation.
If however you do not have a proxy you could remove their permissions from the I.E exe as long as they don't have admin rights to the machine this will scupper their access.
Of course this has a high admin overhead for alot of stations.
Oh and the drive must be NTFS.
