Suggestion, post the URl for the site so we can see whats going on. Also, off the top of my head, most of the email links are just for that, email. You may have to combine forms into database for proper receipt.

Try constructing background stuff in DW, either make form in FW and export/import separately or use one of the extensions available. Also, check your layers properties, may be hidden, lower z value. The text stuff will be better done in DW with an extension, or manually if you know how.

I dont think that the .wav type is your probem. Check the tutorial/extension sites for extensions that simplify this process.
