In my case it was because domain controller loaded before the ISA Server, so it didn't know how to resolve DNS queries. Restarting the DNS service solves this issue.

This one is pretty strange. After a while without using ISA Server to access the Internet, request to access it timeout (from ISA, not from IE).
A simple ipconfig/renew on the ISA server solves this... Why, oh why?
