From the Resources> IP Phones tab check if there's any registered IP set on the system. If so try to deregister it and try to register softphone again. BCM keeps the IP address of the IP sets which uses the license.

Did you try to access it from another PC? If you still have the same screen on another PC, you may want to try accessing over CLI and reboot it from there. Last option is as you said doing a hard boot.
you need to do line tuning for analog trunks. there's an option on analog trunks settings on EM called line tuning digit (0-9) which should be the same as CO site. first set it and reboot the BCM. During boot up it will automatically tune the lines and echo will disappear.
you need to capture sniffer traces from BCM to see which side is dropping the call and what makes it to drop the call after call establishment. to me it seems like a codec negotiation problem. one of the sides may change the codec after call establishment.
I guess the problem is with OS. You may want to try restoring the factory settings just for backup. You can select components to restore when you choose the Restore from Factory Default. Or the other solution is raising a ticket to AVAYA!
press on the services button on the set and check the fw before connecting to BCM. Your set may has a higher fw which doesn't let you downgrade the fw.
