I found the answer to the reporting problem. It was the RevertDN setting in the Meridian Mail Voice Menu Definition. It has to be the same as the incoming queue.

OK. Replacing the handset fixed the login problem, but accepted calls still do not show up on Report 2. I've outted and rebuilt both the queue and the phone twice. Any suggestions for the reporting problem?
