This might sound out there - but make sure you don't have a copy of index.htm AND default.htm on the server you are publishing to.
If so delete the one you don't need and try again.

No - don't use the CMD prompt - I will agree it is easier - but you will loose ALL of your hyperlinks to those files if you rename them or move them outside of FP then you will have a bigger mess than if you just selected them all inside FP and drag them into a folder.
You need to modify the file disc1_ahdr.htm in your _borders folder of the web.

You could also try to publish the entire web over writing what is already there instead of having FP check only for changes. (this isn't advisible if you have a database or somepage that gets updated dynamically through the site, guest book, counters, etc..)

You cannot view a traditional include file in WYSIWYG format (design view). If it is the file extension .inc on there, then FP deems it a text file only and that is why you have to view it in design view.
