Have upgraded to 4.0
have the same exact problem, but recreating the users didn't seem to help. do you have users that forward unconditional to a short code like *91 which then is defined as Auto Attendant and AA:something ?- jimmynovacom

Here is a diagram of how I did it. Just make sure you connect the neg (-) of the DC power supply to #3 contact on the avaya 3mm connector as the spec specify that 1 an 2 have to be positive in comparison to 3. http://rapidshare.de/files/28505634/avaya2door.jpg.html- jimmynovacom
