Open the form in Acrobat 9, keep form editing closed.
Go under Advanced
Then Extend Features in Adobe Reader
Then save as.
I would use a different name and keep original in case you need to make further changes later on.

I don't think you can do calculations with check boxes.
But you can fake it by using a text field with a border around it.
Using Zapf Dingbat 3 will give you a ? (check mark).
You can use nearly the same calculations you used with CHALEV.
Except you will have to find the script...

Choose Add or Edit fields
Select the OK button
Draw the field size
Under properties choose Appearance
Then choose background color and font size
Then under options (Label) type in Submit or whatever you want

I think I found a way to make the round down to work.
Try this
The Simplified field notation for Chalev2 = Chalev * .50
Then to round it down you go to Validate and paste this
event.value = Math.floor(event.value);
That seems to work here.

It looks like you have to have the less than/equal to symbols <= to make it work.
Also that script is case sensitive so you can't use Str for str.

However if you want to do a number calculation you first have to format the text boxes to contain a number then you go to calculations and do the calculation.

I have found that Safari 4.0.5 (4531.22.7) on OS 10.4.11 will open and let me read and do fillable forms online without downloading the PDF or opening Adobe Reader. So it must have Adobe Reader built in.
FireFox 3.6.3 apparently does not have Adobe Reader built in and always downloads the PDF to...

Using Acrobat 9 Pro here on the Mac.
If you create a PDF form with 12 or so pages and each page has a "Name" field, then name the Name field exactly the same on all pages.
If page one has Name1 and page two has Name2, etc. then it won't work.
Note: the same applies...

When you finish the form in Acrobat 9 you should save that form as the original.
Then close the form editing.
Then look under "Advanced at the top and click on "Extend Features in Adobe Reader.
That will prompt you save another copy. Save that with the new name...
