Forms in Word 2000 template 1

Is it possible to (simply) create a form in a Word 2000 template? I have been asked to provide a template containing a form in which the cursor will jump to the next input field following a tab press and in which the form will anticipate the correct format.
 
Hi,

Set up the FIXED TEXT in your form.

Acivate the Forms Toolbar (right click the Word Toolbar & select Forms).

Insert TEXT, DROPDOWN, CHECKBOX etc.

Set parameters on each as needed.

LOCK the form.

[TAB] goes from Form Field to Form Field.

Sorry to tag on to this a bit late but following up the previous answer with a question. I have set up a form as above and can tab between fields but if you hit [enter] then a new line is created which causes problems with form layout. Can this be prevented. TIA
