PIX 506E / TFTP Timed out looking for connection

I am trying to upgrade my pix from 6.1 to 6.22 and I am getting the following error message.

PIX506E# copy tftp flash
Address or name of remote host [192.168.0.4]?
Source file name [c:\]? pix622.bin
copying tftp://192.168.0.4/pix622.bin to flash:image
[yes|no|again]?yes
tftp: Timed out attempting to connect
Image not installed.

Could be a number of things, but here are the most obvious:

1) Have you tired simply pinging the TFTP server IP first to make sure connectivity is OK?
2) Do you have the pix622.bin file in the same directory as your TFTP server, or should another subdirectory be specified when the PIX asks for the file location?
3) Do you have personal firewall software running such as BlackIce or ZoneAlarm? These tend to kill inbound TFTP connections.
 
1) I was able to ping the TFTP server with no problem.
2) The pix622.bin file is in the same directory as my TFTP server.
3) No, I don't have any firewall software running on my TFTP server.
