x450e-48p repeated power failure

Hi all

I have a stack of 8 x450e-48p switches. I have many PCs and VoIp phones connected to them.
Slot 3 has 25 PCs and 13 VoIp phones connected. After ~1 month, the internal power source failed. I was able to run it without trouble using two external power sources, until the replacement arrived.
Yesterday I installed the replacement switch (new unit, not repaired). Over night, this one crashed too, but in a different way - when I arrived in the morning, all leds were flashing green-orange and all PoE power was down. The PCs were still connected to the network.
After a soft reboot and a hard reboot, I still have no PoE on any ports. I installed powere adapters to the phones and they work fine.
The internal power source does not show any error.
When running "show inline-power" on the stack, the command fails when querying slot 3:

My experience with Extreme Networks is short but painful, so any advice is welcome.

Serious error.
All shortcuts have disappeared.
Screen. Mind. Both are blank.
 
Seems like you configured the new switch into the stack wrong I have seen this happen before. It's like the stack MAC is wrong on your new switch.

Like w33mhz said ...Show switch would be helpful as well.
