Wrong Extension Dialed

One of my customers has a strange issue -- occasionally when extension 108 is dialed, extension 118 will ring (and even end up in 118's voicemail). The problem exists on both inbound DID calls and internal station to station calls. Also the problem seems to be intermittent.
I've verified the customer is not mis-dialing, and that there are no coverage paths, shared system access keys or forward buttons on either of the extensions.
My thoughts are ranging from TTRs to cold start/erase/restore. Just wondering if anyone has come across this before or might have some suggestions as to what else I can look at. Thanks in advance.
