Word Questionnaires

I am currently designing a questionnaire in word using a merge with acces which reciptients fill out on the screen and return to me without having to print the questionnaire out.

I have heard that it is possible to have a box like a tick box where an x can be entered into or writing if the question is ooen ended.

On;y problem is I have no idea how this can be done can anybody enlighten me to a way that something like this can be done.......please!!!!

Select View>Toolbars>Forms. This brings up the Forms toolbar, which is what you're looking for. just click on the item you want and click on your form to put it there. Double-click on the item to bring up an option box so you can make additional changes.

You probably want to use a table to control the placement of everything (otherwise, it has a tendancy to move around when some fills the information out).

After you're done, select Tool>Protect Document. Choose Forms. This basically allows the user to complete the information but not make changes to the form itself. You don't need to have a password (if you use one, make sure you remember what it is!).
