Word/PDF style Table of Contents

Is there an easy way to create a Word/PDF style Table of Contents for a web site. I need to use the file title, and the headings within each page (from <H1> to <H7>) to build the tree, which should ideally have click expanders (+/-). I've tried Index Generator by Mark Howard, which is great - but doesn't do the headings (only looks at file titles), I've also looked at an AWK script (which does the headings-but that's all). If there's no easy way, is there a hard way!

Any ideas greatly appreciated.
