Word/Outlook spell check formatting

I have a user who's spell check in Word and Outlook is being disabled by the "Do not check spelling or grammar" format option under Tools > Language > Set Language. It doesn't happen all of the time. I know how to correct it but not how to prevent it. Any ideas? Could there be a problem with a template or just some default settings?
