Word/Outlook/Exchange Error

One of the users here at my company has an error crop up occasionally that I can't explain/trouble shoot. Our server is running Windows NT 4.0/Exchange 5.0 and the client machine is Windows 95. Every once in a while in Outlook 98, if the user tries to access a Word attachment in a message, they will get the following error:

Closing the error screen causes Outlook to close, but Windows 95 doesn't seem to be affected. Also, Outlook can immediately be restarted without a problem, and the offending attachment can usually be opened without a problem. It only happens every once in a while and under different conditions (early morning, late in the day, with no other programs running, with several running, etc.). It's not a huge problem, but it is an annoyance, and if I can fix it, it would make my week.

Anyone have any ideas?
