Word Add-in problem

I want to create a Word Add-in that creates a dynamic list of options very much like SYNONYMS.

I can update the SYNONYMS pop-up menu using "AddinInstance_OnConnection" etc

and I can create dynamic menu options
using Word macro and "App_WindowBeforeRightClick"

I can't seem to connect the 2.

Does someone have a sample application that
emulates the SYNONYMS pop-up menu
and can be distributed as a DLL / Add-in?
