Word 2000 Toolbars

Can any one tell me what is going wrong and how to stop it.
We have many templates the people use and with some when a new document is created from the template the toolbar at the top of the word form is missing. This will happen with the particuar template all the time.

Do you have the latest service patch? Maybe this will help.
