Won't create new user account properly

I tried to create a new account earlier and while it created the account, it failed while trying to create the user directory. It gave me the message to have the user visit

http://OURDOMAIN-SB1/ConnectComputer

to finish the setup.

What's the common reason for this?

Are u using a 3rd party/homegrown tool to create the user accounts (I have seen similar issues)? Sounds like the tool is not working correctly, since this error is not commonly found in Windows

No, just going in through server management / users. Could it be a license issue. We may be at or above our max.
