Wiring for QPC237 4-Wire E&M Card

Trying to find the NTP or cabling diagram for a QPC237 4-Wire E&M Card. Specifically need to know how ports 0 and 1 are layed down coming off the 25 pair cable.

Actually looking for the original NTP's for an old Nortel SL1NT. Need to know how the card slots are wired out on an old QSP35/36 PE shelf.
