I have one Windows 2003 server to which I lose the ability to remote desktop when patched.

After the reboot it not longer allows me to connect, to rectify the problem I have to login locally and deselect the Enable Remote Desktop check-box, click the apply button, the re-select the check box, upon a reboot of the server the remote desktop functionality returns.

This is the only server we experience this on, but we have configured all our servers the same?

When I used to manage Win2K3 servers specifically 64-bit servers this would occur on any particular server. No rhyme no reason. Never did figure out what was causing this. The only solution was to reboot the server again remotely
