Win2K dropping off Ultrium drives

We have multiple servers on the SAN backing up on ADIC tape library with multiple Ultrium drives using CommVault Galaxy.

We're experiencing situations where Win2K drops off the drives while backup is running. Galaxy will show that the drives are offline and when we check Device Manager, the drives are missing from the list.

Re-scan the devices may or may not bring up the drives again.

Any advice/help would be appreciated.

What make and model Host Bus Adapters are you using?

We're using Compaq LP7000/LP8000 HBA on Compaq Proliant servers.
Additional info:
What we notice is that IBM Ultrium driver we installed do not carry digital signature. Does this matter?

What version of firmware are the HBA's and which driver are you using. If you would like, send me a screen shot of your emulex config utility screen and I will exam it for you.
