White text "ghosting" over an image

Several times now this has occured: I have PDF'd (at press quality 2400dpi etc)& adverts I have created that have a white text over an image look perfectly normal - ie. all text can be read. When this PDF goes to the printers, and the magazine comes back from the printers, the adverts come back with the white text being "ghosted" or barely there.

Basically a document (magazine) leaves me - the designer looking exactly as it should yet when it returns from our printers, white text over an image doesnt print properly. Has anybody got any ideas? I work on InDesign CS2 & do not have the overprint stroke ticked in the attributes.
 
Do the overprint preview or separation preview functions of InDesign offer any insight?
 
Sometimes it's a RIP that can't handle some pdf - basically transparency like the transparent text box over the color background. I still always use compatiable with pdf 4 for everyting and have not had problems. In other words, I flatten everything.

You also might want to check the transparency flattener settings in the advanced section of pdf export or the Print window (if you're printing a ps file for distiller) and make sure that high is selected.
