Where is the bottleneck?

How many bottlenecks are there to the loading of a web page?

I can identify three, but there are probably more:
1. The design of the page can cause it to load slow.
2. The server that supplies the page can be overloaded.
3. The ISP that calls the page can be choked up.

Is there any way of identifying where the bottleneck is?

I often suspect my ISP because the same page, same server, at different times performs at 1/100th the throughput. If I could prove it, I would change my ISP.
