A custom form was created with outlook and published to the exchange server. Somehow it got deleted and i want to restore it from backup and was wondering where on the server it Stores a form for a public contact list??

Forms are typically saved to a special container in the mailbox of the user who published the form to the server: the Personal Forms Library. So if you want to restore the forms, you actually have to restore the user's mailbox.

Before you move from there, I have to ask, what container did you usually look in to use the form before it got deleted? That will determine if the form was in the PFL or not.

In any case, your form won't be recoverable as a distinct file, it was buried within an .edb file, either priv.edb or public.edb. Restoring it might cause more trouble than finding a way to recreate it.
