when I do a save to file and write to flash it

Yes, when you choose Save Configuration and New Flash Image, you need to give the read only and read/write passwords again. This also reboots the Firebox. If you choose Save Configuration File Only, you do not have to give the passwords again, and depending on what you have changed through Policy Manager the changes can be applied without rebooting the firebox.
