root will have a cron entry like this:

fbcheck:23456789:wait:/usr/sbin/fbcheck 2>&1 | alog -tboot > /dev/console # run /etc/firstboot

fbcheck is just a shell script which checks for the existence of /etc/firstboot and runs it if it exists.
fbcheck renames /etc/firstboot so it only runs once - on the 'first' or 'next' reboot only.

So, /etc/firstboot (if it exists) is a script to run at the next reboot only and is called by the fbcheck entry in /etc/inittab .
